Pet Prime Rebate Instructions

Thank you for recent membership to Pet Prime! Zoetis, a pet medicine and vaccination company, offers a rebate to all new Pet Prime members. Please follow the steps below to request your rebate:

1. Go to zoetispetcare.com/rewards
2. Click: Join Rewards
3. Enter Your Email Address
4. Create a Password
5. Click: Sign Up
6. Add Information to Create Your Account
   1. First Name, Last Name, Zip Code
   2. Click: Sign Up
7. Add Pet
   • Pet Name, Species, Sex, Breed, Birthday
   • Click: Save & Continue
   • Click: I’m Done (If you signed 2 or more pet’s up, click: Add Another Pet and proceed)
     ** In 24 hours, you must check your email for a rewards email to verify your account. You will not receive rebates until you finsih this one-time step.**
8. Click: Submit Invoice
9. Take Picture of Invoice w/ Mobile Device and/or Save to Computer
10. Click: Choose File and Add Invoice Picture
11. Enter: Purchase Date
12. Select: Add a Vet Practice
    • a. Enter Zip Code for Clinic at Which You Signed Up on Pet Prime
    • b. Select: Correct Clinic
13. Select Drop Down on Purchase Location: Vet Practice
14. Click: Continue
15. Select: Your Pet
16. Click on the Preventative Included in your Plan
    • Select Dosage (make sure to select the correct size)
    • Select 12 doses
    • Enter your Invoice TOTAL for Amount Paid
17. Repeat the above steps if more than one preventative is included in your plan
    • (i.e. Proheart & Simparica)
18. Click: Submit Invoice
19. Click: Yes – I’m Done
20. Sign Out
